Is hospital discharge service right for me?
Our Hospital Discharge service is for those individuals who are:
- Currently in hospital but are soon to be discharged home.
- Due to be admitted to hospital for a major procedure and will require support on their discharge.
- A hospital discharged patient still in need of support following support from a family member who is now due to go back to work.
- Patient discharged home as independent, but only to find themselves struggling after a few days.
